WHAT IS PCOS-A condition in which ovaries of a woman fail to release eggs regularly.
A woman’s ovary is designed in such a way that under the effect of hormones released by brain FSH and LH, her ovary will produce eggs every month in interval of 28 to 30 days approximately. While during process of releasing egg, ovary releases two hormones,1] Estrogen[E2] under influence of FSH and P4, under effect of LH. This mechanism is popularly 2 cell 2 hormone theory.
This hormone E2 is responsible for all the important function in a women body, like sharp brain, good pumping of heart, strengthening of her muscles, deposition of calcium in bone, good quality hair, skin tone, sexual desire, orgasm etc. In short a woman is woman because of E2. When ovary stop producing E2, a woman reaches menopause-fragile bones, skin wrinkles, loss of memory, vaginal dryness, lack of libido etc.
This E2 also makes a layer inside uterus known as endometrium, a sort of bed making for embryo implantation. This E2 is secreted for 1st 14 days of menstrual cycle alone. Egg get released on 12th to 14th day of cycle, which also marks the release of hormone progesterone P4, for next 14 days it is coupled with production of hormone progesterone produced by ovary under effect of LH secreted by brain. This P4 opposes E2. It stops the growth of endometrium, makes it more nutritious for embryo. This hampering of endometrial growth is important as unrestricted proliferation of endometrium will predispose to heavy menstrual blood loss and in future may lead to endometrial cancer.
Thus an endometrium is made in 28 days under the effect of E2 and P4 during the process of maturing and release of egg from ovary.
But if egg is not fertilized by sperm it gets destroyed -hormone E2 P4 stop, endometrium sheds off which is seen as menstrual bleeding.
Any ovary which is not able to release egg on time, will contain many follicles containing eggs at a time and when ultrasonography is done it shows- multiple follicles containing ovary -called POLYCYSTIC OVARY.
SIGNS AND SYMPTOMS OF PCOS-
1)Irregular menstrual cycle-28-30 days cycle is no more. Only E2 is released which continues making endometrium as long as possible as no P4 as it is produced only after release of egg. She can menstruate once in 15 days ,45 days ,2 -3 months, no fixed pattern.
2)Heavy or scanty blood flow-No control of endometrial growth so heavy flow. PCO ovary can also produce male hormone testosterone which negates effect of E2 leading to scanty menses.
3)Excessive hair growth on face and body, acne, scalp hair loss under effect of testosterone.
4)INFERTILITY-no eggs no baby.
IS IT CURABLE -NO
Usually it is genetic condition, symptoms can be treated using medication for producing egg in particular cycle, stopping excessive bleeding using hemostatic drugs.
